Fees 
$150 per initial, intake, hour (couples or individual)

$105 per individual, therapy hour & $125 per couple or family session hour.

 

Sessions generally run around 50 minutes. The remainder of the allotted hour is used to write notes for your treatment. Initial intake sessions include the development of an assessment, and treatment planning.

Coaching rates are similar, however bundled sessions may be purchased for a lower, total cost. 

Payment 
Credit Card, HSA/FSA Card, Zelle, Check or Cash 
All major credit cards are accepted & payment is due at the time of service.

Cancellation Policy 
I work by scheduled appointment. Please provide at least 24 hours advanced notice of cancellation to avoid a charge for your missed session. Those who fail to notify me of a needed cancellation may be subject to a charge of half of their session price. 

Insurance 
I am not a contracted provider on any insurance panels & will not deal directly with any insurance plans. 
Clients may try to file a claim directly with their provider ... & may be able to get partial reimbursement should your insurance plan provide out-of-network coverage. 

 These are the general steps to pursue this option : 

1. Call the mental health number on the back of your insurance card to verify if you have "Out of Network" coverage and the amount. 

2. Verify your Deductable, Co-Pay, and any Session Limits. 

3. Ask your carrier to clarify & specify the information required to submit a claim ... and verify if you need to use a particular form provided by the insurance company. 

4. Get the Name and Address of the representative to whom you will need to submit the Claim Form. 

5. Get a Pre-Authorization number as needed. 

6. Inquire if you can use a universal insurance claim form ... this is called a "CMS 1500 Claim Form". 

7. You may submit the Claim directly to your insurance company based on the information obtained in the steps above. 

 

Phone Calls 
There is no fee for routine phone calls up to 15 minutes to address administrative or scheduling matters. However, there is a 
$1.00/minute charge for calls beyond 15 minutes which address therapeutic or coaching issues
